From the Editor-in-Chief his last issue for 2012 is closing the 9th year of activity of Clinical Cases in Mineral and Bone Metabolism (CCMBM) and we are very proud of this success. The Journal is internationally recognized and its popularity is increasing, with an even brighter future as the Impact Factor will be obtained. This number of CCMBM is presenting a unique collection of reports in the area of orthopaedics and metabolic bone diseases. The role of leptin in regulating bone mass is mastered by Siswo Legiran and Maria Luisa Brandi. The importance of the interaction between fat and bone tissues is described, along with the role of gene polymorphisms in the leptin cascade cassette. Ranuccio Nuti is reporting on the impact of calcium supplementation on cardiovascular risk, an area quite debated today. In the successive article the results on the applications of FRAX® test in patients included in controlled clinical trials are elegantly presented by Carmelo Elio Fiore. Stefania Ciolli is offering an overview on the bone complications in multiple myeloma patients. The area of shoulder injuries in soccer players is well reviewed by experts in the field. Similarly, Francesco S. Martelli et al. beautifully reviewed the pathogenetic bases of periodontal disease, reporting data from a large cohort of Italian patients. Four articles are dedicated to osteoporosis drugs in osteoporosis, beginning with an interesting article on adherence to calcium and vitamin D treatment evaluated in an Italian multicenter survey carried out in osteoporotic patients. Giovanni Iolascon et al. report on a prospective pilot study on the effectiveness of teriparatide in The clasped hands, Auguste Rodin. the control of back pain. The role of bisphosphonates to induce bone osteonecrosis and of strontium ranelate to heal subtrochanteric atypical femural fractures are described in two successive articles. The Editor-in-Chief is presenting the importance of guidelines in the management of osteoporosis with a focus on prevention through lifestyle and pharmacological interventions, keeping in mind the problem of adherence and the opportunities offered by polytherapies. Liria Papa et al. report on a randomized controlled clinical trial on the effectiveness of osteopathic manipulative treatment in reducing pain and improving the quality of life in elderly patients affected by osteoporosis. In three reports dedicated to orthopaedic surgery kyphoplasty, femur fractures intervention and surgical approaches in Osteogenesis Imperfecta are described in detail. Two interesting clinical cases, one on misdiagnosis in child abuse versus Osteogenesis Imperfecta and one on double osteomalacia are presented respectively by Patrizia D’Eufemia et al. and by Kamal K. Pandita et al. Finally, Francesco Tonelli is entertaining us again with an in memoriam article for Renzo Mantero, a scientist, a surgeon and a man of unique human quality and volcanic energy.
