La sorveglianza sanitaria:
SORVEGLIANZA
in ambito sanitario
Sistematica, attiva, quotidiana osservazione
dell’occorrenza e della distribuzione della malattia
in una popolazione e degli eventi o condizioni che
aumentano o diminuiscono il rischio di
occorrenza della malattia stessa.
Sorveglianza in medicina
Continuo e sistematico processo di raccolta,
analisi, interpretazione e circolazione delle
informazioni ottenute finalizzato al
monitoraggio dei problemi inerenti la salute
Epidemiologia descrittiva, analitica, sperimentale
Sistema di sorveglianza ideale (I)
• Semplice: nei metodi di raccolta dati, nell’analisi, management ecircolazione dei risultati raccolti, nel suo stesso mantenimento nel tempo. Ruolo crcuciale delle case definitions
• Flessibile: capace di rispondere in modo rapido a nuovi problemi, adattabile ai miglioramenti tecnologici e alle nuove definizioni di caso, in grado di essere integrabile con altri sistemi.
• Attendibile: i dati devono essere completi e validi. Questo richiede l’utilizzo per la loro raccolta di personale sanitario professionista adeguatamente formato.
Sistema di sorveglianza ideale (II)
• Condiviso: necessità di condividere razionale, strategie e obiettivi tra tutto il personale ospedaliero (criticità nell’attuare sistemi di sorveglianza in piccoli ospedali, etc.).
• Rapido: la tempestività è un punto cruciale non solo riguardo all’identificazione dei casi ma anche nel favorire il ritorno dei dati alle strutture coinvolte nell’attività di sorveglianza (feed back).
• Confrontabile: i risultati devono essere esportabili ad altri contesti e paragonabili con risultati ottenuti in altre popolazioni.
• Affidabile: il sistema deve essere in grado di raccogliere, elaborare, analizzare una mole consistente di dati, con un basso margine d’errore.

Surveillance System - NNIS (USA)
• Il più antico sistema di sorveglianza delle infezioni nosocomiali a livellonazionale del mondo (1970).
• Network di 300 ospedali con n° letti > 100, a partecipazione volontaria. Soprattutto ospedali affiliati all’Università con un n° medio di 360 letti in degenza e 38 letti in ICU.
• Campione rappresentativo del 5% degli ospedali di primo soccorso (acute care hospitals) in USA.
• Hospital-wide surveillance in prima fase.
• Attualmente, attività mirata alle infezioni acqusite in ICUs e a quelle della ferita chirurgica (SSI).
• Definizioni di HAI standard utilizzate dagli Infection Control Practitioners (ICPs).
• Quantificazione del rischio per ogni ICU coinvolta, attraverso l’utilizzo di tassi specifici.

Epidemiologia delle I.O. negli USA
Il Centre for Disease Control (CDC) stima che le infezioni
nosocomiali contribuiscano dallo 0,7 al 10,1% delle morti totali
negli States e dallo 0,1 al 4,4% delle morti avvenute in ambito
ospedaliero (
MMWR, 1992
).
Come detto, il fenomeno si ripercuote anche in termini di costi
sanitari, quantificabili in un aumento della spesa dai 5 ai 10
miliardi di dollari americani.
Questi dati non stupiscono, se si pensa che fino al 20% dei
40.000.000 di pazienti operati annualmente negli USA presenta
almeno un episodio di infezione ospedaliera nel periodo post
operatorio (
Horan TC et Al., Infect. Contr. Hosp. Epidemiol.,
1993
).

– Lo studio SENIC ha in particolare dimostrato che, in assenza di misure di controllo, l’incidenza delle Infezioni Ospedaliere tende ad aumentare: negli Ospedali statunitensi dove non erano attivati programmi di controllo delle infezioni, l’incidenza delle infezioni è aumentata del 18% nello spazio di 5 anni

Significativa riduzione (1990-1999) della frequenza di
batteriemie: 44% in UTI non chirurgiche, 43% UTI
Il rischio di acquisire un’infezione ospedaliera in
Italia è, in media, della stessa entità o maggiore di
quanto riportato in letteratura
• La frequenza di IO è sovrapponibile a quella rilevata dal
sistema di sorveglianza statunitense (NNIS) negli ultimi
30 anni o da studi multicentrici in Paesi del Nord Europa.
• Le caratteristiche epidemiologiche (localizzazioni più
frequenti, patogeni coinvolti, procedure e pazienti a
rischio) sono simili a quelle descritte dal NNIS.
• Ogni anno si verificano in Italia 450.000-700.000
infezioni (incidenza 5-8%) che sono direttamente
responsabili di 4.500-7.000 decessi.
•
Se si assume che il 30% siano prevenibili…..ogni anno sono
potenzialmente prevenibili 135.000-210.000 infezioni e
1.350-2.100 decessi.

Perché un Sistema di Sorveglianza Sindromica?
Criticità dei Sistemi di Sorveglianza Classici (I)
I sistemi di sorveglianza delle malattie sono classicamente basati
sulla segnalazione di informazioni che riguardano pazienti con
diagnosi definite.
Per molte malattie, però, la diagnosi clinica presenta margini di
incertezza, o il quadro clinico è comune a infezioni causate da
numerosi microrganismi.
Per questo, i sistemi di sorveglianza classici presentano limiti di
tempestività e sensibilità, caratteristiche indispensabili sia per
identificare malattie la cui causa potrebbe inizialmente essere
sconosciuta, sia per mettere in atto quelle misure di sanità pubblica
volte al contenimento della diffusione dell’infezione.
Perché un Sistema di Sorveglianza Sindromica?
Criticità dei Sistemi di Sorveglianza Classici (II)
Per questi motivi, sono stati da tempo avviati nuovi sistemi di
sorveglianza, basati non più sulla diagnosi di malattia, ma sulla presenza
di un insieme di segni e sintomi, che costituiscono una sindrome.
I recenti episodi di allarme causati da malattie trasmissibili emergenti,
come la SARS o da casi umani di virus influenzali a potenzialità
pandemica, i timori legati a episodi di bioterrorismo o il ritardo con cui
vengono segnalati e caratterizzati episodi epidemici, hanno evidenziato, a
livello internazionale, la necessità di realizzare sistemi di sorveglianza e
risposta rapidi nei confronti di eventi acuti o inattesi, potenzialmente
pericolosi per la salute pubblica.

Impiego di piattaforme elettroniche e Web per la raccolta dei dati ed il
feed-back informativo;
Scelta di fonti dei dati di sorveglianza che permettano un rapido
rilevamento del dato non atteso; i.e. l’assenteismo scolastico/lavorativo o
la vendita dei farmaci sono indicatori precoci vs i dati di dimissione
ospedaliera.
Sensibilità e Specificità
Mancanza di dati di sensibilità e specificità dei sistemi di sorveglianza
sindromica: solo 3 dei 115 sistemi riportano una valutazione di questi
indicatori di performance;
Eventi troppo rari per una valutazione sul campo;
Mancanza di metodi standard per la valutazione delle performance (ad
esempio l’epidemia influenzale stagionale non è adatta alla valutazione
di episodi di malattie respiratoria da virus emergenti).

NELL’AREA METROPOLITANA GENOVESE
Obiettivi.
Obiettivo primario del progetto è l’organizzazione e la
sperimentazione di un sistema di sorveglianza sindromica, che attraverso
l’analisi dei dati di accettazione dei Pronto Soccorso dell’area
metropolitana genovese, consenta di
rilevare tempestivamente
- cluster di infezioni acute febbrili dell’apparato respiratorio
(influenza-like illness, ILI; bronchioliti e polmoniti)
- cluster di gastroenteriti non emorragiche
- casi di sospetta epatite acuta
- casi di rash accompagnati da febbre secondo definizioni di caso
standardizzate

competenti, tra cui il Dipartimento Salute e Servizi Sociale
della Regione Liguria, i Dipartimenti di Prevenzione
delle A.S.L., Ospedali, ecc.
attivare le misure di sanità pubblica volte al contenimento
dell’infezione
Perché Concentrarsi su Questi Quadri Sindromici
Rappresentano l’esordio clinico:
delle infezioni a trasmissione aerea o feco-orale a maggior rischio epidemico sia nella comunità, che in ambiente ospedaliero; in particolare, esse riconoscono quale principali target di popolazione la fascia pediatrica, con elevati rischi di ospedalizzazione nei primi 5 anni di vita e costituiscono, inoltre, importanti cause di morbosità e mortalità nei soggetti fragili;
di malattie infettive da importazione per il viaggiatore ed il migrante (epatite A, diarree ad etiologia virale, ecc);
di malattie infettive esantematiche per cui è previsto il piano nazionale di eliminazione (morbillo, rosolia, ecc);
di epatiti acute B che, in un quadro epidemiologico caratterizzato dalla vaccinazione universale nelle classi con età inferiore a 25 anni e l’immunizzazione dei gruppi ad elevato rischio, rappresentano un evento sentinella in ambito vaccinale;
di infezioni da microrganismi che si ipotizza possano essere impiegati per attacco bioterroristico.
Outcome del Sistema di Sorveglianza Sindromica
valutare l’impatto che le infezioni acute dell’apparato respiratorio (ILI, bronchioliti e polmoniti) e le gastroenteriti hanno sulle strutture dei Dipartimenti di Emergenza e Accettazione;
stimare l’incidenza delle epatiti acute;

Medium: level of influenza activity usually seen when influenza virus is circulating in the country based on historical data

* Baseline influenza activity is the level that clinical influenza activity remains in throughout the summer and most of the winter. Usually, there will be a 6-12 week period in winter when the level of clinical influenza activity rises above the baseline threshold, but in the very occasional winter (perhaps 1 in 10) activity never gets above the baseline level.

Acute sudden-onset affection with > 38°C fever accompanied by at least one of the following symptoms: Headache, generalised discomfort, fever sensation (sweating, shivers), asthenia
and by at least one of the following respiratory symptoms: Cough, pharyngodynia, nasal congestion.
For the clinical diagnosis of influenza in children it is important to consider what is specified for adults bearing in mind that:
1) younger children cannot describe the systemic symptoms, which can instead appear with: Irritability, crying, loss of appetite
2) in nursing babies influenza is often accompanied by vomiting and diarrhoea and only exceptionally by fever; 3) red eyes and conjunctivitis are characteristic of influenza in pre-school children, in case of high fever;
4) in 1-5-year-old children the influenza syndrome is frequently associated with laryngotracheitis, bronchitis and high fever.

The strains isolated suggested that the outbreak was attributable to the Fujian-like strain of influenza A/H3N2/Genoa/3/04, which characterized the 2003–2004 influenza season in Italy but was not included in the 2003–2004 vaccine. Vaccination was ineffective because of the mismatch between wild and vaccine strains.

Ostreopsis ovata is an epiphytic
potentially toxic dinoflagellate
• Ostreopsis ovata is a marine epiphytic potentially toxic dinoflagellate with a world-wide distribution, usually living in tropical and subtropical areas, but recently found with more and more frequency in the Mediterranean sea. It is normally associated with other epiphytic or benthic dinoflagellates (Taylor 1979, Tognetto et al. 1995, Faust et al. 1996, Chang et al. 2000, Vila et al. 2001, Sansoni et al 2003).
• During recent years, Ostreopsis spp. produced blooms in the Tyrrhenian and Southern Adriatic Sea. Recently, there has been the first record of Ostreopsis cfr. ovata in the Northern Adriatic Sea from macroalgae in two areas, the Gulf of Trieste (Italy) and close to Rovinj (Croatia) (Monti M et al. Mar Pollut Bull. 2007).
• The blooms occur probably due to favorable and stable weather conditions for many days, when water temperature and barometric pressure are high and hydrodynamism is low.

and human health:
what known?
• Data appear to be lacking regarding the effects on human health of
Ostreopsis ovata.
• Some Ostreopsis strains produce a highly toxic compound known as palytoxin and analogues which can accumulate in fish and are implicated in clupeotoxism, associated with eating clupeoid fish. Environmental conditions and factors promoting palytoxin production are still largely unknown (Sansoni et al 2003, Tanuyama et al. 2003, Usami et al. 1995).
• Palytoxin is one of the most potent non-peptidic marine toxins. It has a lethal dose to 50% of rats, mice, dogs and monkeys when given intravenously at ranges between 0.03 and 0.45µg/kg. It causes cytolysis as a result of the inhibition of Na+, K+-ATPase and has been recognised as the causative agent of fatal human seafood poisoning (Onuma Y et al. Toxin 1999, 4 Tosteson MT. Food Science and Technology Series 2000).
O. Ovata and human health:
the recent italian experiences
• During recent years, some reports registered Ostreopsis spp. blooms occurring in the Tyrrhenian and southern Adriatic Sea have been related to human health problems, such as breathing and skin irritation, in tourists attending the beaches and inhaling marine aerosols. (Sansoni G et al. Biologia Ambientale 2003, Di Turi et al. Biol Mar Medit 2003, Gallitelli M et al. JAMA 2005, Ciminiello P et al. Anal Chem 2006, Brescianini C et al. Eurosurveillance 2006 ).
• Some authors had previously described other Ostreopsis species algal blooms along the Tyrrhenian coast, suggesting similar symptoms in exposed persons (Tognetto L et al. Botanica Marina 1995, Sansoni et al. 2000).
• The symptoms associated with O. ovata blooms in most of the
affected Italian Regions are quite similar to those observed in the United States (Florida and other areas of the Gulf of Mexico), but the events in the United States have been associated with blooms of another algae, Karenia brevis, which produces brevetoxins (Mini-monograph: brevetoxins. Environ Health Perspect 2005; 113 (5): 618-657).
• During 2003 (mid-August, 7 days) and 2004 (early September 5 days) 2 microalgal blooms of O. Ovata occurred along the coasts of Bari, Italy.
• Twenty-eight cases (9 men, 18 women, and 1 child; mean age, 38.6 years, 10-68 years) presenting symptoms such as copious watery rhinorrhea, dry or mildly productive cough, fever to 38°C and bronchoconstriction with mild dyspnea were identified in subjects exposed to marine aerosol during recreational or working activities on the beach.
“……Although there was no experimental challenge of healthy volunteers exposed to marine aerosol with or without Ostreopsis blooms or isolation of a putative palitoxin, complaints occurred concurrent with the algal blooms and disappeared when the Ostreopsis population decreased.
Most symptoms improved or disappeared very rapidly when the patients left the beach.
It is likely that the patients’ symptoms were related to the algal proliferation……”

In mid July 2005, clinical-epidemiological and local environmental investigations were started following the first warnings of an unusual number of people needing care in several city Hospital Emergency Rooms (E.-R.s) in Genoa, after recreational or working activities on the beach.
Symptoms registered in more than 200 patients who attended E.R.s were rhinorrhoea, cough, fever, bronchoconstriction with mild breathing difficulties, wheezing, and, in a few cases, conjunctivitis.
Clinical-epidemiological investigation on human
cases: case-definition of “algal syndrome”
A clinical-epidemiological case-definition was achieved following collection of the most significant anamnestic and clinical data obtained from the patients, including:
- those who had been by the seaside (<300 ft from the shore-line) and who needed care in an E.-R.
presenting with the association of at least two of the following symptoms: - cough - dyspnea - pharyngeal pain - rhinorrhea - fever >38°C - headache - lacrimation - nausea/vomiting.
Materials and Methods
Clinical-epidemiological data were collected by MDs, during the outbreak, from patients with attendance to the E.R. Hospitals who matched the
above described
case-definition, using both hospital clinical diaries and an “ad hoc” questionairre .

Laboratory analysis results were obtained during the acute phase in 82 (39.2%) patients:
The blood count showed leucocytosis (mean white cell count 13,900/mm3±3,400; range
10,100-23,900) and neutrophilia (mean 82.2%±4.7; range 75.2-91.5) in 46.3% and 40.2% of the cases, respectively.
No other significant impairment of other biohumoral parameters was recorded (normal values of transaminases, gamma-glutamyl transpeptidase, creatinine and sedimentation rate).

Just in the same days of the outbreak in humans, a concomitant superficial proliferation of macroalgal mucilage was described in the tract of the coast where the most cases have occurred.
Local environmental protection staff immediately analysed air and water samples to exclude the presence of chemical pollution, and suspected a particular kind of unicellular alga, Ostreopsis spp, as a causative agent.
Sea water samples from around five beaches were taken for analysis every day in the first week after the outbreak, then twice a week and finally once a week. In the first days after the alert many samples were found to have high densities of Ostreopsis ovata, up to several thousands cells/l of sea water and hundreds of thousands of cells/g on macroalgae samples.
The analysis of water, plankton and macrophytae samples showed the presence of ‘putative palytoxin’ because the test could also have detected an isomer of palytoxin (Ciminiello P et al. Anal Chem 2006)
After this bloom, O. ovata density dropped to much lower levels in a few days, and no further human health effects were recorded.

An high water temperature and barometric pressure and a no windy meteorological condition, with a flat sea, in the days just before the first registered cases, matched with the peculiar typology of the coast with numerous small inlets, represent factors which have certainly favoured the algal bloom.
Public Health control measures adopted
following the 2005 outbreak
During 2005 outbreak, on the basis of the results of both the clinical-epidemiological and environmental investigations the Mayor of the city of Genoa, following the recommendations of the local Public Health Unit, suddenly ordered forbiddance of bathing along the tract of the coast interested by the outbreak.
This safety precaution was adopted on 18th July, after the recognition of the outbreak, and went on for only 3 days due to the simultaneous resolution of the emergency for human health.
The local Public Health Unit and the Regional Liguria Agency for the Environmental Protection faced the media, giving real-time informations to the population on both the developing situation and the Public Health preventive measures to adopt.
An algal syndromic network, including the Regional Epidemiological Observatory, the main city Hospital E.-R.s and the local Public Health Units was set up and continued surveillance, throughout the summers of 2005 and 2006.

During the next summer (July 29th -August 3rd and August 21st-23rd, 2006), 19 cases were suddenly identified by the surveillance network, following the above described case-definition, in the cities of Genoa and La Spezia. On the basis of the experience of the previous year, the Genoese local Public Health Authorities ordered bathing to be forbidden (from 29th July to 4th August).
The anamnestic and clinical pictures of these patients were almost superimposable to that observed in the previous year but no patient required hospitalization.
Lesson learned from the recent
experience and open issues (I)
• Worrying episodes occurred in Italy in the last years, describing humans affected mainly by respiratory symptoms after the exposure to concomitant Ostreopsis Ovata blooms, as in our experience, undoubtedly has opened the discussion on the urgent need to monitor and prevent such events.
• The importance of it is not simply related to an overriding Public Health issue but also to both social and economic aspects, strictly related to these phenomenons.
• We support the hypotesis that Ostreopsis Ovata could have been the causative agent of these epidemics, mainly in consideration of the clear existing causative temporal criterion and by the exclusion of other plausible factors or conditions that have been investigated.
• Despite this, new studies focusing both the pathogenesis and the epidemiology of the disease but also the geographical distribution of the alga and its related favouring conditions to spread are necessary, also to identify new possible future preventive strategies.
The setting up of a syndromic surveillance network, including the Regional Epidemiological Observatory, the main city Hospital E.-R.s and the local Public Health Units could represent an efficacious tool for both the rapid detection of the sentinel-cases and health regulations by the Local Public Health Authorities, i.e. forbid bathing in the area, to limit the burden on human health. The description of the clinical syndrome reported by patients exposed to Ostreopsis Ovata bloom could contribute not only to a better understanding of the effects of the exposure but also to the setting up of a more stringent case-definition for syndromic surveillance purposes.
