Analisi semiologica retrospettiva in cani affetti da shunt
portosistemico
RIASSUNTO

Lo shunt portosistemico (PSS) è una malformazione vascolare che determina il dirottamento del sangue proveniente dalla circolazione venosa addominale alla circolazione venosa sistemica, baypassando i capillari sinusoidi epatici ed il parenchima epatico. L’aspecificità dei sintomi con cui si manifesta la malattia risulta essere la problematica principale nella diagnostica del PSS; infatti spesso la diagnosi può sfuggire, o perchè i segni clinici sono estremamente lievi o perchè il proprietario ritiene che certi comportamenti ormai “abituali” siano normali (Jonson, 2002). Questo studio è indirizzato verso l’individuazione dei principali sintomi e segni clinici che caratterizzano il quadro clinico di animali affetti da shunt portosistemico congenito e acquisito e verso il riconoscimento delle alterazioni ematobiochimiche e di funzionalità epatica presenti. I sintomi più rappresentati sono stati, crisi convulsive (37,5%), apatia (25%), scialorrea (25%) e vomito episodico (18,75%). Nel 31,25% dei cani era presente microepatia. Le alterazioni ematobiochimiche più frequenti sono state : aumento degli acidi biliari (nel 92,3%), iperammoniemia (72%) e ipoalbuminemia (68,75%). Nel nostro studio a guidare il percorso diagnostico verso test di funzionalità epatica, è stata primariamente la consapevolezza che determinate razze (Yorkshire terrier, Schnauzer nano, Maltese ecc.), unitamente alla giovane età e alla presenza di una sintomatologia lieve e fluttuante, fossero predisposte allo sviluppo della malattia.

Portosystemic shunts (PSS) are vascular anomalies that redirect blood from the portal vein to the systemic circulation, bypassing the hepatic sinusoids and liver parenchyma. The nonspecific simptomatology of the desease manifestation is the main problem for the diagnosis of PSS; actually the diagnosis can be missed or because the clinical signs may be vague or because the owner considers normal some behaviours now become "usual" (Jonson 2002). This study is directed to recognize the main simptoms and signs characterizing the clinical presentation of animals affecdted by congenital or acquired portosystemic shunt (PSS) and also to recognize the serum biochemical abnormalities and the alterations of liver function. The most represented simptoms have been: seizures (37,5%), apathy (25%), hypersalivation (25%), and episodic vomit (18,75%). In the 31,25% of the dogs there was microepatia. The most frequent biochemical abnormalities and the alterations of liver functions have been: increasing of serum bile acids (92,3%), iperammoniemia (72%) and ipoalbuminemia (68,75%). In our study to lead the diagnostic way to effect tests of hepatic functionality has been primarily the awareness that particular breeds (Yorkshire terrier, miniature Schnauzer, Maltese etc.), together with young age and milder and fluctuant simptomatology, were predisposed to develope the desease
