MODIFICAZIONI EPIGENETICHE NELLA ScS 3
CXCL4 → could possibly function as an epigenetic imprinting factor during the differentiation of monocytes into monocyte-derived dendritic cells (moDCs) and further increase the inflammatory responses
Ah Kioon, M. D. et al. Sci. Transl Med. (2018) Silva-Cardoso, S. C. et al. J. Immunol. (2017) Silva-Cardoso, S. C. et al. Front. Immunol. (2020)
Cytokine & Growth Factor Reviews 22 (2011) 1–18
«….CXCL4 amplifies IFN-I production by breaking immune tollerance to self-DNA […] ….. CXCL4 is both a chaperone,
because it protects DNA from degradation and facilitates internalization of DNA into pDCs, and an adjuvant because it renders DNA hyperstimulatory for TLR9.»
«Levels of CXCL4 are elevated in SSc patients, especially early
SSc…[…]…CXCL4 strongly correlates with both skin and pulmonary disease but also appears to predict progression in systemic sclerosis phenotypes»

PERICITES
ENDOTHELIAL-TO- MESENCHYMAL TRANSITION
• EndoMT occurs both in physiological and pathological events and may play a role in the pathogenesis of certain human fibrotic and vascular disorders
Hinz B. et al. Am. J. Pathol. 2012, 180, 1340–1355.
Mendoza FA et al Arthritis Rheumatol. 2016 January ; 68(1): 210–217.
• Multiple pathways, such as Trasforming Growth Factor b (TGFb) and Endothelin-1 (ET-1), may participate in the molecular mechanisms of the EndoMT process
From L. Pérez, et al. Cytokine Growth Factor Rev (2016)
ENDOTHELIAL-TO- MESENCHYMAL TRANSITION IN ScS
“Cells in intermediate stages of EndoMT were identified in dermal vessels of either patients with SSc or bleomycin-induced and urokinase-type plasminogen activator receptor (uPAR)- deficient mouse models.”
“SSc-dMVECs exhibited a spindle-shaped appearance, co-expression of lower levels of CD31 and VE-cadherin with myofibroblast markers (α-SMA+ stress fibres, S100A4 and type I collagen).”
“Treatment of H-dMVECs either with SSc sera or TGFb1 resulted in the acquisition of a myofibroblast-like morphology and contractile phenotype and downregulation of endothelial markers in parallel with the induction of mesenchymal markers.”
Jimenez SA. ISRN Rheumatol. 2013 Sep 23.
ENDOTHELIAL-TO-MESENCHYMAL TRANSITION AND EPCs IN ScS
“Cultured mononuclear cells from SSc patients displayed higher abundances of CD31 and aSMA”
“Circulating eEPCs were significantly lower in all patients with SSc, both in limited and diffuse SSc (lSSc/dSSc)”
To assess phenotypic characteristics of EPCs from SSc patients and from patients with Very Early Diagnosis of SSc (VEDOSS) compared to healthy controls (HC) EPCs.
To evaluate the expression of a-smooth muscle actin (α-SMA) as marker of a pro- mesenchymal switch (EndoMT)

FARMACI BIOLOGICI E ANTIFIBROTICI
NELLA SCLEROSI SISTEMICA
RITUXIMAB NELLA SCLEROSI SISTEMICA
Twenty-four studies:
11 RCT 9 cohort 4 case series
PROS
✓ The use of RTX in SSc is supported by the pathophisiological aspects of the disease
✓ The meta-analysis demonstrated that RTX improved FVC from baseline at 6 and 12 months
✓ These results were supported by a commensurate decrease of mRSS
✓ The effect of RTX appeared to be superior compared to other IS drugs
CON
✓ The FVC improvement may not fully reflect clinically significant changes and a wide variability in within-person often exists
✓ The effect of RTX on DLCO was not stable in the sensitivity analysis and compared with the controls
✓ There was very high eterogeneity in the matched control analysis, making interpretation difficult
✓ Data are inconcluding in terms of infectious adverse events
CONCLUSIONS
✓ The effect of RTX should be regarded with caution (small number of cases; various treatment regimens; short duration of FU; not uniform data)
✓ It is reasonable to infer that RTX is associated with stability of ILD in the short term and with a modest improvement
✓ No significant differences compared with MMF was noted
✓ The degree of improvement noted in the different studies needs to be corroborated by a robustly designed RTC Goswami RP 2020 Moradzadeh M 2021 Gkiouras K 2021

NELLA SCLEROSI SISTEMICA
CONCLUSIONI
