An Illustrated History
A
history of medicine and oncology from its origins to the present day – as envisaged in this eleven-volume project – has never before been written. By placing medicine and oncology into the broader framework of general history and humanist studies this project not only presents the facts but also their significance, thus providing valuable information about the major role played by medical sciences in the development of humanity. Although medical literature contains excellent, but albeit outdated histories of medicine, there are no books on the global history of oncology, in particular, ones that provide a comprehensive overview of the integration between these two disciplines. Starting with the origins of life and diseases, including cancer, this work takes the reader on a fascinating journey regarding the many practices and medical concepts adopted by ancient civilisations, and the transformations that occurred in the Graeco-Roman world, in Islamic culture and the entire western hemisphere. The descriptions of the diseases, medical theories, and therapies of the past are placed in their historical and social context, highlighting the most important advances in medicine, surgery, pharmacology and oncology. From the 20th century onwards, the narration is dominated by cancer in its etiological, evolutionary and therapeutic aspects, along with their impact on the contemporary society. From the discovery of the structure of DNA to modern precision oncology, scientists searched, found and analysed; they theorised and experimented. They dared. Their revolutionary discoveries have completely changed the way in which we interpret a morbid phenomenon that sparked enormous interest throughout the ages but for a very long time remained relatively unknown. The consequences in therapeutic terms came relatively quickly, leading to the cure for several cancers and paving the way to eradicate others. Research, however, will never stop and the final part of the work focuses on the future perspectives, providing the conceptual bases required to understand their logical evolution.Scientifically accurate, fluidly written, accessible to all, the work is embellished with numerous illustrations ranging from the ancient graffiti found in caves to the images of the molecular circuits used by cancer cells. They provide a visual representation of the evolution of medicine and oncology and facilitate their comprehension. The books in this project are not only
authoritative texts available for consultation by scholars of medicine and oncology. They also provide interested readers the opportunity to trace the evolution of modern ideas regarding health and the treatment of diseases, and to find the correct answer to the many questions that crowd the minds of those who

na storia della medicina e dell’oncologia dalle origini ai giorni nostri, come immaginata in questo progetto, non è stata mai tentata in precedenza. L’intera opera non si limita a raccontare i fatti ma il significato dei fatti, ponendo la medicina e l’oncologia nell’ampio contesto della storia generale e delle discipline umanistiche, per dare preziose indicazioni sul ruolo importane svolto dalle scienze mediche nel progresso dell’umanità. Pregevoli storie della medicina, sebbene datate, sono rinvenibili nella letteratura medica, mentre sono assenti libri di storia globale dell’oncologia e in particolare quelli ideati per dare una veduta esaustiva dell’integrazione tra le due discipline. Partendo dalle origini della vita e delle malattie, compreso il cancro, l’opera – in undici volumi – conduce il lettore in un viaggio affascinante tra i molteplici aspetti della pratica e dei concetti medici delle civiltà antiche e le trasformazioni intercorse nel mondo greco- romano, nella cultura islamica e nell’intero Occidente. La descrizione delle malattie, delle teorie mediche e delle terapie del passato è posta nel contesto storico e sociale in cui si sono affermate, mettendo in rilievo i più importanti avanzamenti verificatisi in medicina, chirurgia, farmacologia e oncologia. A partire dal XX secolo è la concezione della malattia neoplastica a dominare la narrazione, considerandone gli aspetti etiologici, evolutivi e terapeutici e il loro impatto nel contesto della società contemporanea. Dalla scoperta della struttura del DNA alla moderna oncologia di precisione, gli uomini di scienza hanno cercato, trovato e analizzato; ipotizzato e sperimentato. Hanno osato. Le loro scoperte rivoluzionarie hanno fatto cambiare completamente il modo di interpretare un fenomeno morboso che, pur suscitando grande interesse in ogni epoca, è rimasto troppo a lungo poco conosciuto. Le conseguenze in termini terapeutici non si sono fatte attendere e hanno consentito di guarire diverse neoplasie e creare i presupposti per debellarne altre.La ricerca, tuttavia, non avrà mai termine e la parte finale dell’opera è rivolta alle prospettive future, fornendo le basi concettuali per comprenderne l’evoluzione logica. Scritta con rigore scientifico e con stile fluente, accessibile a tutti, l’opera è impreziosita da numerose figure, che a partire dagli antichi graffiti delle caverne fino alle immagini dei circuiti molecolari utilizzati dalla cellula cancerosa, forniscono una rappresentazione visiva di come si siano evolute medicina e oncologia, facilitandone nel contempo la comprensione. Così concepita, essa non è solamente un autorevole testo di
consultazione per coloro che si dedicano allo studio della medicina e dell’oncologia, ma offre ai lettori interessati l’opportunità di seguire la formazione delle idee moderne sullo stato di salute e sul trattamento delle malattie, e di trovare una risposta corretta ai numerosi quesiti che affollano la mente di coloro che in numero crescente rivolgono la loro attenzione ai tumori.

professor Massimo Lopez –
SHORT CVMedical oncologist, lives in Rome.
His curriculum studiorum began with a degree in Medicine (Rome, 1967, “Sapienza”
University) with a thesis on certain rare skin cancers. He went on to obtain a specialisation degree in Cardiology (Turin, 1971), Oncology (Roma, 1973) and Internal Medicine (Rome, 1976). In 1979 he obtained his National Qualification as Head of Department in Oncology.
Apart from the work done at the Santa Maria Goretti Hospital in Latina (1986-1989) as Head of the Medical Oncology Division, he carried out his activity at “Regina Elena”
Institute for the Study and Treatment of Cancer in Rome, initially as a medical assistant in the Pain Treatment Centre (1970), then as assistant in the Medicine and Haematology Division, and later as chief resident in the same Division, renamed Medical Oncology.
He was then the Director of the Division of Medical Oncology B from 1990 to 2011, the year he retired. During that same period he taught Cancer Pharmacology and Chemotherapy and later also Medical Oncology at the 1st School of Specialisation in Oncology, “Sapienza” University.
For many years he was the oncology consultant of the Forensic Medicine Service and Drug Commission of the Ministry of Health and Presidency of the Republic.
Member of numerous Scientific Societies in Italy (Associazione Italiana di Oncologia Medica - AIOM; Società Italiana Tumori - SIT) and abroad (European Society of Medical Oncology - ESMO; American Society of Clinical Oncology - ASCO; New York Academy of Sciences), he is currently a Permanent Member of ASCO. Member of the Scientific Committee of the European Journal of Oncology and Editor of Oncology Care Today &
Tomorrow; from 1998 to 2016 he was the Scientific Director of the journal La Clinica Terapeutica.
During his professional career he has always focused in particular on clinical research in oncology, teaching, and the dissemination of information.
In the period 1970-1990 his research activities focused primarily on the study of new anticancer drugs, biomodulation and novel combination chemotherapy regimens in different advanced solid cancers such as breast cancer, gastric carcinoma, melanoma, non-Hodgkin lymphomas, colorectal cancers and soft tissue sarcomas. He won international renown for his work on several treatments: identification of a new hormone therapy for male breast carcinoma using antiandrogens; the development of the FAB combination in the treatment of gastric cancer; the EDIC regimen in soft tissue sarcomas and various antiproliferative agents in non-Hodgkin lymphomas, melanoma and female breast cancer. Studies on the use of epirubicin and idarubicin in the treatment of female breast cancer have contributed significantly to the registration of these anthracyclines in the therapy of advanced stages of this cancer, respectively in the United States and Italy.
Between 1990 and 2000, his most important research studies focused on: the immunotherapy of kidney cancer; the immunochemotherapy of lung and colorectal cancer; overcoming drug resistance in breast cancer and soft tissue sarcomas;
amelioration and prevention of anthracycline-induced cardiotoxicity; adjuvant and neoadjuvant treatment of breast cancer and adjuvant therapy of colorectal carcinoma.
From 2000 onwards, while continuing his clinical research on innovative treatments of breast cancer (using agents such as taxanes, vinorelbine and gemcitabine) and the clinical evaluation of new drugs, he also systemised the huge mass of information regarding molecular cancer therapy and designed and conducted translational research studies with molecularly targeted agents alone or in combination with traditional chemotherapy agents.
He has also significantly inputted into research regarding the prevention and early detection of cancer combined with optimal organisational criteria.
Between 1986 and 1989 he launched a cancer prevention programme in the province of Latina successfully involving the population due to the accuracy and rapidity of the implementation of the services envisaged by the programme. This initiative was extremely useful when he developed a completely computerised cancer prevention project later efficiently used to organise and direct a national cancer prevention programme for FISDE (Supplementary Health Fund for ENEL employees).
In 2005 he was awarded the Order of Merit of the Italian Republic (Grand Officer) by the President of the Italian Republic. In 2016 he was appointed Academician of the Accademia Lancisiana by the Minister of Cultural Heritage and Activities.
In 1985 he initiated the publication of a series of monographs on cancer – the Collana di Oncologia published by the Società Editrice Universo, Rome. In 2000 the first edition of Oncologia Medica Pratica became the reference text for most Italian oncologists.
He also wrote the entire medical oncology section in the Trattato Italiano di Medicina Interna published by Teodori, and was co-editor of the last edition of this classic text, published in 2004. Since 1998 he has co-edited the Prontuario Terapeutico Universo and has written the entire section dedicated to medical oncology.
Thousands of copies were printed of the following more informative books: Cancro.

In more recent years he supplemented the Oncologia Medica Practica by focusing on the study of historical sources of medical and oncological knowledge; his goal was to present the conceptual foundations of Medicine and Oncology as a helpful tool for both doctors and members of the public interested in this topic. This sparked a much broader literary endeavour entitled Medicina e Oncologia. Storia illustrata. [Medicine and Oncology. An Illustrated History]. During the drafting of the eleven volumes in Italian and English, he became aware of the growing interest of laypersons in their own health and issues such as cancer, considered up until recently, a taboo. Proof comes in
the form of the numerous television programmes that deal with cancer and the enormous number of magazines and specialised articles on this issue.
The request for more information became gradually more pressing and the virtual audience of readers increased enormously to include people who, having once broken the glass ceiling that kept them away from this kind of literature, now want to obtain a correct and accessible information which has to be, however, scrupulous, thorough, and easy to understand. Illness, including cancer, always seems to lurk in the shadows, and even those who are not doctors feel the need to understand how, when and why humans fall sick, which weapons are at their disposal in order not to succumb, and what are our future prospects.

CURRICULUM SINTETICOOncologo medico, vive a Roma.
Il suo curriculum studiorum inizia con la laurea in Medicina e Chirurgia conseguita a Roma nel 1967 all’Università “Sapienza” con una tesi su particolari neoplasie cutanee, e prosegue negli anni successivi con i diplomi di specializzazione in Cardiologia (Torino, 1971), Oncologia (Roma, 1973) e Medicina Interna (Roma, 1976). Nel 1979 ha conseguito l’Idoneità Nazionale a Primario in Oncologia.
Con una parentesi (1986-1989) all’Ospedale Santa Maria Goretti di Latina – dove è stato Primario della Divisione di Oncologia Medica – ha svolto la sua attività professiona- le all’Istituto “Regina Elena” per lo Studio e la Cura dei Tumori di Roma, iniziata come assistente medico del Servizio di Terapia del Dolore (1970) e proseguita come assistente della Divisione di Medicina e Ematologia, aiuto della stessa Divisione denominata in se- guito di Oncologia Medica, fino al ruolo di Direttore della Divisione di Oncologia Medica B dal 1990 al 2011, anno del suo collocamento in quiescenza. Nello stesso periodo è stato incaricato dell’Insegnamento di Farmacologia e Chemioterapia dei Tumori e, poi, anche di Oncologia Medica, nella 1a Scuola di Specializzazione in Oncologia dell’Università
“Sapienza”.
Per molti anni è stato consulente oncologo del Servizio di Medicina Legale e della Commissione Unica del Farmaco del Ministero della Sanità, e della Presidenza della Re- pubblica.
Membro di numerose Società Scientifiche italiane (Associazione Italiana di Oncologia Medica - AIOM; Società Italiana Tumori - SIT) e straniere (European Society of Medical Oncology - ESMO; American Society of Clinical Oncology - ASCO; New York Academy of Sciences), è attualmente Permanent Member of ASCO. Membro del Comitato Scientifico dell’European Journal of Oncology ed Editor di Oncology Care Today & Tomorrow, dal 1998 al 2016 è stato Direttore Scientifico della rivista La Clinica Terapeutica.
Nel corso della sua carriera professionale, ha sempre dedicato una particolare attenzio- ne alla ricerca clinica in oncologia e ad attività didattiche e divulgative.
L’attività di ricerca è stata prevalentemente orientata, negli anni 1970-1990, verso lo studio di nuovi farmaci antitumorali, biomodulatori e associazioni chemioterapiche origina- li in diversi tumori solidi in fase avanzata, quali il carcinoma della mammella, il carcinoma gastrico, il melanoma, i linfomi non-Hodgkin, i tumori colorettali e i sarcomi dei tessuti molli. Risonanza internazionale hanno avuto l’individuazione di una nuova terapia ormona- le del carcinoma della mammella maschile con antiandrogeni, lo sviluppo dell’associazione FAB nella terapia del cancro dello stomaco, del regime EDIC nei sarcomi dei tessuti molli e di vari agenti antiproliferativi nei linfomi non-Hodgkin, nel melanoma e nel cancro della mammella femminile. In quest’ultima neoplasia, gli studi effettuati con epirubicina e ida- rubicina hanno contribuito significativamente alla registrazione di queste antracicline nella terapia della malattia in fase avanzata, rispettivamente, negli Stati Uniti ed in Italia.
Negli anni 1990-2000, le linee di ricerca più importanti hanno riguardato l’immuno- terapia del carcinoma renale, l’immunochemioterapia dei tumori del polmone e del co- lon-retto, il superamento della farmacoresistenza nel carcinoma mammario e nei sarcomi dei tessuti molli, l’attenuazione e la prevenzione della cardiotossicità da antracicline, il trattamento adiuvante e neoadiuvante del cancro della mammella e la terapia adiuvante del carcinoma colorettale.
Dal 2000 in poi, pur continuando l’attività di ricerca clinica su trattamenti innovati- vi dei tumori della mammella (attraverso l’impiego di agenti quali taxani, vinorelbina e gemcitabina) e sulla valutazione di nuovi farmaci, ha provveduto a sistematizzare l’enorme numero di informazioni sulla terapia molecolare del cancro e a disegnare e condurre studi di ricerca traslazionale con agenti a bersaglio molecolare in associazione o meno ad agenti chemioterapici tradizionali.
Anche nel campo della prevenzione e della diagnosi precoce dei tumori, ha dato signi- ficativi contributi alla ricerca, non disgiunta da criteri organizzativi ottimali.
Negli anni 1986-1989 ha implementato un programma di prevenzione oncologica nella provincia di Latina che ha coinvolto positivamente la popolazione per l’accuratezza e la rapidità con cui venivano effettuate le prestazioni previste dal programma. Questa espe- rienza si è rivelata di grande utilità per la messa a punto di un progetto di prevenzione oncologica completamente informatizzato, utilizzato in seguito in maniera efficiente per organizzare e dirigere un programma nazionale di prevenzione dei tumori per il FISDE (Fondo Integrativo Sanitario per i Dipendenti dell’ENEL).
Nel 2005 il Presidente della Repubblica Italiana gli ha conferito l’onorificenza di Gran- de Ufficiale al Merito della Repubblica. Nel 2016 è stato nominato Accademico dell’Acca- demia Lancisiana dal Ministro dei Beni e delle Attività Culturali e del Turismo.
Nel 1985 ha iniziato la pubblicazione di una serie di volumi monografici di pertinenza oncologica, che costituiscono una Collana di Oncologia edita dalla Società Editrice Uni- verso, Roma. Nel 2000 è comparsa la prima edizione di Oncologia Medica Pratica, diven- tato il testo di riferimento della maggior parte degli oncologi medici italiani.
Autore dell’intera parte di oncologia medica del Trattato Italiano di Medicina Interna del Teodori, è anche co-editor dell’ultima edizione di questa opera classica, pubblicata nel 2004.
Dal 1998 è stato co-editor del Prontuario Terapeutico Universo, di cui ha scritto l’intera parte dedicata all’oncologia medica.
Di carattere divulgativo sono stati i libri Cancro. Conoscerlo per imparare a sconfiggerlo (2003 e 2007), I tumori della mammella (2005) e GIST. Tumori Stromali Gastrointestinali (2005 e 2009), stampati in decine di migliaia di copie.
Negli anni più recenti, come complemento dell’Oncologia Medica Pratica, si è dedicato allo studio delle fonti storiche dello scibile medico e oncologico con l’obiettivo di presen- tare la Medicina e l’Oncologia nei loro fondamenti concettuali per essere di supporto sia ai medici che al pubblico interessato all’argomento. Ne è nato un progetto di ampio respiro,
intitolato Medicina e Oncologia. Storia illustrata. Nel corso della stesura, sia in italiano che in inglese degli undici volumi di cui l’opera si compone, ha preso atto della sempre maggiore attenzione del comune cittadino verso la sua salute e dell’interesse nei confronti di argomenti come il cancro, fino a qualche tempo fa considerato un tabù. Ne sono prova quotidiana le numerose rubriche televisive e la grande mole di riviste e di articoli specia- lizzati sull’argomento.
La domanda di essere informati si è fatta sempre più insistente e la virtuale platea di lettori si è allargata, accogliendo anche coloro che, abbattuto l’invisibile muro di cristallo che li teneva lontani da questa letteratura, vogliono una informazione corretta e accessibi- le, rigorosa e nel contempo facile da assimilare. Le malattie, compreso il cancro, sembrano essere costantemente in agguato e anche coloro che non sono medici, sentono l’esigenza di capire come, quando e perché esse incontrano l’umanità, di quali armi questa dispone per non soccombere e quali sono le prospettive future.
