Io curo i MALATI!

Appropriatezza in oncologia (e non solo!)
• Appropriatezza Scientifica

Pemetrexed in Nonsquamous Non–Small-Cell Lung Cancer
The Billion Dollar Subgroup Analysis
Cisplatin-pemetrexed is considered the standard of care for lung adenocarcinoma by all major oncology
guidelines.
The basis for this recommendation is a 2008 randomized trial that was designed to test the non-inferiority of
cisplatin-pemetrexed vs cisplatin gemcitabine in the
overall NSCLC patient population (
GV Scagliotti, et al., J Clin Oncol. 2008;26(21):3543-3551).

Pemetrexed in Nonsquamous Non–Small-Cell Lung Cancer
The Billion Dollar Subgroup Analysis
The Scagliotti’s study found similar OS between the
groups: 10.3months, establishing non-inferiority (hazard ratio [HR], 0.94;95% CI,0.84-1.05; upper margin for
noninferiority, 1.18).
However, the authors analyzed survival differences in 19 subgroups, including histology, and found that for
adenocarcinoma OS was significantly increased with cisplatin-pemetrexed combination vs cisplatin-
gemcitabineby 1.7 months(12.6 vs
10.9months;HR,0.84;P = .03). Forsquamous lung cancer, the trend was opposite (9.4 v 10.8 months;HR, 1.23; P = .05).

Pemetrexed in Nonsquamous Non–Small-Cell Lung Cancer
The Billion Dollar Subgroup Analysis
• While pemetrexed improved OS in the nonsquamous group (but not squamous group) it did so without an improvement in progression-free survival (PFS).
• there is no reason for a drug like pemetrexed (unlike immunotherapy) to increase OS without increasing PFS. Empirical data confirm this
event is unlikely.
• The study did not adjust for multiple hypothesis testing, and the authors conducted at least 19 subgroup analyses.
• subgroup analyses should always be considered hypothesis generating and not definitive.
• Furthermore, when multiple subgroup analyses are performed and 1 or 2 comparisons produce significant results, it is highly likely that those positive findings are due to chance alone.

We cannot incorporate drugs as expensive as pemetrexed into our guidelines or hail them as superior in the absence of robust scientific evidence that supports those claims.

Appropriatezza scientifica
• Il trattamento convenzionale:
– Come trasferire schemi e dosaggi nella pratica clinica?
• Il trattamento convenzionale:
– L’ultimo lavoro pubblicato?
• La prescrizione off –label :

What is Standard of Care?
• It is the degree of prudence and caution
required of an individual who is under a duty of care.
• A duty of care is a legal obligation imposed on an individual requiring that they adhere to a standard of reasonable care while performing any acts that could possibly harm others.

Off-Label Use of Anticancer Therapies
• To determine the most effective treatment, physicians must consider
– the type of cancer,
– the unique presentation and history and symptoms of each patient,
– the stage of cancer,
– the available and accessible therapeutic alternatives.
• Because cancer can often be life threatening, advances in clinical management outpace the drug approval process.
23
John E. Feldmann, 2005
Investigator-Sponsored Trial
• Investigator-Sponsored Trials (ISTs) may include
studies done with an investigational agent, which is defined as an agent or formulation that has not been approved for marketing by the appropriate
Regulatory Agency.
• Must be considered investigational a pharmaceutical form of an active ingredient with a marketing
authorization, when used or assembled in a way
different from the approved form, or when used for an unapproved indication, or when used to gain
further information about an approved use.

Which changes in clinical outcomes are clinically relevant?
• Whether a treatment result is of clinical
relevance is a value judgement and depends on the perspective
• A multitude of different stakeholders
(oncologists, regulatory agencies, patients,
payers, society, industry) might have different views
• Is a 20% increase in overall survival equally clinically relevant for all cancer, treatment lines, patient populations?
26
Britta Paschen, 2007
Factors determining clinical relevance
• The seriousness of the disease or condition
• Intent of treatment (palliative, curative, preventive)
• The availability and performance of other alternative therapies
• The nature of the target population and whether it includes vulnerable populations, such as children and the elderly
• The choice of endpoints (response, progression-free survival, overall survival, QoL)
• Magnitude of clinical benefit
• related to the current QoL or remaining life expectancy
• Risk-Benefit ratio
• Individual patient characteristics (co-morbidities, age, other prognostic factors)
27
Britta Paschen, 2007
Drug prescription appropriateness in the elderly
• Drug prescription inappropriateness increases the prevalence of drug-induced harms, with consequences related to
patients’ health and cost of care.
• Polypharmacy and inappropriate medication prescriptions are associated with increased morbidity and mortality.
• This is particularly true for elderly people, whose frailty makes the choice of the correct drug prescription even more
difficult.
