• Marzo 2014: riscontro visivo-palpatorio di tumefazione in loggia tiroidea dx
• Familiarità positiva per CTD (madre: riscontro occasionale di microcarcinoma papillare in TXT per GMN)
• In anamnesi:
– SM in tp con IFN
– Dislipidemia in atorvastatina 10 mg – Ipertensione arteriosa in ramipril 5 mg
• TSH regolare, AbTg e AbTPO negativi.
• Ecografia tiroidea: lobo dx: complesso nodulare costituito da nodulo disomogeneo paracarotideo di 14x16x24 mm e nodulo ipoecogeno con cercine calcifico incompleto di 12x12x19 mm. Nulla a sx.

Indicazioni alla FDG-PET nello staging iniziale
• 18FDG-PET scanning may also be considered as :
- a part of initial staging in poorly differentiated thyroid cancers and invasive Hürthle cell carcinomas, especially those with other evidence of disease on imaging or because of elevated serum Tg levels
- a prognostic tool in patients with metastatic disease to identify lesions and patients at highest risk for rapid disease progression and disease-specific mortality

Indicazioni all’impiego di TC/RM nello staging iniziale
(• Cross-sectional imaging of the neck and upper chest (CT, MRI) with IV contrast should be considered
– in the setting of bulky and widely distributed recurrent nodal disease where US may not completely delineate disease,
– in the assessment of possible invasive disease where potential aerodigestive tract invasion requires complete assessment, or
– when neck US is felt to be inadequately visualizing possible neck nodal disease (high Tg, negative neck US).

recommendation, Low-quality evidence)• Empiric (100–200 mCi) or dosimetrically determined RAI therapy may be considered in patients with more significantly elevated serum Tg levels, rapidly rising serum Tg levels, or rising ant-Tg antibody levels, in whom imaging has failed. If empiric RAI is given and the posttherapy scan is negative, the patient should be considered to have RAI-refractory disease and no further RAI therapy should be administered. • Seconda radioterapia metabolica mediante somministrazione di 100 mCi di I131 previa stimolazione con TSH endogeno (sospensione LT4).
• 20/07/2015: TSH = 60,73 uU/ml, Tg = 23,2 ng/ml, AbTg negativi
• WBS post-dose: scomparsa dei residui di parenchima captante segnalati nel precedente controllo del 2014 in corrispondenza della loggia tiroidea.
• All’emitorace dx anteriormente e inferiormente, si osserva piccola area di fissazione del radioiodio compatibile con una localizzazione secondaria di malattia (verosimilmente ultime coste di dx).
• Nulla di patologico a carico dei restanti segmenti corporei.

• PET-FDG: l’indagine tomoscintigrafica globale corporea non ha dimostrato la presenza di aree di patologico accumulo del radiofarmaco da riferire fondatamente a lesioni ad elevata attività metabolica.
• CONCLUSIONI: assenza di reperti patologici ipermetabolici.

Indicazioni alla FDG-PET nel follow-up
• 18FDG-PET scanning should be considered in high risk DTC
patients with elevated serum Tg (generally >10 ng/mL) with negative RAI imaging.In patients with a TSH-stimulated Tg ≤ 10 ng/mL, the sensitivity of
18FDG is low. 18FDG-PET should be considered only in DTC patients with a stimulated Tg level >10 ng/mL. This level needs to be adapted and lowered in case of aggressive variant of thyroid cancer that may
produce low amounts of serum Tg and in in patients with undetectable Tg but with persistent Tg antibodies.
The sensitivity of 18FDG-PET scanning may be slightly increased with TSH stimulation

Indicazioni all’impiego di TC/RM nel follow-up
• CT imaging of the chest without IV contrast (imaging pulmonary parenchyma) or with IV contrast (to include the mediastinum) should be considered in high risk DTC patients with elevated serum Tg (generally >10 ng/mL) or rising Tg antibodies with or without negative RAI maging.
• Imaging of other organs including MRI brain, MR skeletal survey, and/or CT or MRI of the abdomen should be considered in high-risk DTC patients with elevated serum Tg (generally >10 ng/mL) and negative neck and chest imaging who have symptoms referable to those organs

TSH suppression in the long-term follow-up
• In patients with a biochemical incomplete response to therapy, the serum TSH should be maintained between 0.1 and 0.5 mU/L, taking into account the initial ATA risk classification, Tg level, Tg trend over time, and risk of TSH suppression.

• PET-FDG previa stimolazione con rhTSH: tenue iperfissazione del radiofarmaco a carico di una piccola adenopatia di 11 mm di aspetto ovalare in sede paratracheale alta di sx. Il reperto, di lieve entità, non raggiunge al momento chiari criteri di significatività.
• Nulla altro da segnalare a livello dei restanti distretti corporei esaminati.
• 20/02/2017: TSH = 0,091 uU/ml Tg = 4,04 ng/ml, AbTg negativi
• 22/02/2017: TSH = 147 uU/ml
• 24/02/2017: Tg= 6,34 ng/ml
• Il dato clinico-strumentale orienta per persistenza biochimica di malattia in assenza di recidiva strutturale documentabile.

Using response to therapy to guide disease long-term surveillance and therapeutic management decisions
• Biochemical incomplete response with stable or declining serum Tg values should lead to continued observation with ongoing TSH suppression in most patients. Rising Tg or anti- Tg antibody values should prompt additional imaging and potentially additional therapies.
• A structural incomplete response may lead to additional treatments or ongoing observation depending on multiple clinico-pathologic factors including the size, location, rate of growth, RAI avidity, 18FDG avidity, and specific pathology of the structural lesions

for suspected structural neck recurrence?
• Therapeutic compartmental central and/or lateral neck dissection in a previously operated compartment, sparing uninvolved vital structures, should be performed for patients with biopsy-proven persistent or recurrent disease for central neck nodes ≥8mm and lateral neck nodes ≥10mm (Strong recommendation, Moderate-quality evidence)
The decision for treatment is best derived through collaborative team approach involving surgery, endocrinology, the patient and family. Therefore, cytologic confirmation of disease can be deferred if the findings of the FNA will not lead to additional evaluation or treatment
