ATTACHMENT TO THE APPLICATION OF THE ERP TENDER YEAR 2011 1. Copy of a valid identity card, front and back;
2. For the non-EU citizens copy of the residence charter or residence permit with 24 month validity;
3. Copy of the income tax return of the family for the year 2010 (attach copy of the mod. 730, Unico, CUD forms);
4. In case of children not yet born attach copy of the pregnancy condition documentation;
5. If in the family there is a person over 60 years of age non self-sufficient attach copy of the certification by the Medical Commission of the Local Health Centre;
6. If in the family there is a disabled person attach copy of the certification by the Medical Commission of the Local Health Centre;
7. In case of accommodation in inappropriate house or provided for as temporary by the public assistance attach declaration by the Local Health Centre or Municipality;
8. In case there are architectonic barriers in the housing occupied by persons with mobility disability attach Local Health Centre certification;
9. In case of dwelling in overcrowded house attach Local Health Centre certification;
10. In case of house in unsanitary housing attach Local Health Centre certification;
11. In case of accommodation in house that must be released attach:
• Enforceable order of eviction;
• (or) verbal legal settlement;
• (or) evacuation order;
• (or) retirement order of public or private employee that utilises a service house;
• (or) to be in the objective condition that makes the use of the house impossible.
12 If the applicant has not had a house for at least one year attach the documentation that demonstrates the lack of a house and the reason.
