5.1.4. vHIT come indice prognostico del recupero post Disturbo Vestibolare Acuto

Attualmente stiamo valutando se la funzionalità del VOR possa essere messa in relazione con il recupero del paziente e “se ad un recupero di funzione del VOR dinamico corrisponda effettivamente un ripristino della performance posturale del paziente e viceversa”. Poiché è noto che il fenomeno del compenso post-DisturboVestibolareAcuto si realizza a frequenze diverse, che sono descritte patologie in cui il danno è frequenza-dipendente come la Malattia di Ménière, che l'HF-VOR sembra recuperare più lentamente rispetto al LF-VOR studiabile con il BVC e che è stata già descritta una buona correlazione tra cHIT e prognosi e normalizzazione dei sintomi (Mandalà M et al, 2008), è possibile che proprio l’analisi della misura della funzionalità del VOR, eseguita con vHIT, possa essere la più pertinente, in quanto esprime in maniera migliore di altri esami vestibolometrici l’effettivo recupero del paziente post-DVA e può assumere, inoltre, un ruolo prognostico costituendo un’indagine che, laddove patologica in fase cronica, motiva l’avvio del paziente ad un programma rieducativo.
