Background: to evaluate the lymphocyte population, PD-1, PDL-1 and TREM-1
expression to better understand the immunophenotype of HPV+
OP-SCC Methods:30 specimens of advanced stage OP-OP-SCC resected between 2004 and 2012; exclusion criteria: bony invasion, recurrent/metastatic disease, previous RT/CT for HNSCC, previous CT for other site solid and/or hematological tumors and immunosuppressive therapy. IHC for p16 and E6, CD4, CD8, PD-1, PDL-1 and
TREM-1 was performed on consecutive sections. Here we report data about first 17 cases. Results: p16 was positive in 60% of pts(24%+;36%++); E6 was positive in
100% of pts(59%+;41%++); ITE: CD4 was positive in 100% of pts(12%+;24%+ +;64%+++) as CD8(12%+;36%++;53%+++), PD-1 was negative in 29% and positive in 71% of pts(71%+), PDL-1 on tumoral cells was positive in 100% of pts(53%+;48%++), TREM-1 was negative in 82% and positive in 18%(12%+;6%+ +); PTE: CD4 was positive in 100% of pts(6%+;24%++;71%+++) as CD8(48%+; 41%++; 12%+++), PD-1 expression was negative in 59% and positive in 41% of pts(41%+), PDL-1 expression was positive in 100% of pts(6% +;59%++;36%+++) TREM-1 was negative in 18% and positive in 82%(64%+;18%++). McNemar test showed a concordant expression of CD4, CD8, and PDL-1 both in ITE and PTE, while TREM-1 was more expressed in PTE(p = 0.001) and PD-1 in ITE(p =
0.12) Conclusions: For the first time, we described a high expression of TREM-1 in the PTE and this finding could have a clinical relevance because TREM-1 triggering reverses the M2-polarizing effect of hypoxia imparting a M1-skewed
