provide good resolution. In our opinion, these units will make prehospital ultrasound diagnoses easier, i.e., in ambulances, helicopters or airplanes, wherever every square centimeter counts
1. In the hospital, the »pocket« ultrasound unit has its draw- backs:
1. Its small size makes it an easy prey to theft, potentially a serious problem. If the solution to this problem is to set this miniature unit up on a cart, all the advantages of miniaturization immediately vanish, and only the drawbacks remain (small screen, poor resolution, etc.).
2. In the hospital, we need a minimum of material.
The contact product, the disinfection product, the material for emergency interventional pro- cedures, and material for medicolegal purposes (i.e., a hard copy record) are used constantly.
Moreover, the device must be set on a stable surface, not be the bed itself. In addition, the larger the screen, the greater the visual comfort.
Without all these features, a portable appara- tus is not at all practical to use in the hospital, and a cart is, in practice, necessary. The cart cancels the advantage of miniaturization. With- out a high-quality image, only the drawbacks remain.
To sum up, in the hospital, a choice must be made between cumbersome and small units, but we believe that choosing between small and ultra- small units would be highly questionable.
The ideal ultrasound device must respond to pre- cise requirements, including simplicity of use, com- pactness, and mobility. It must be small, equipped with only one probe, and should privilege the two- dimensional.
The Central Unit
The wide range of new equipment available (Doppler, continuous or pulsed, not to mention color and invasive echography) has made the units cumbersome, complex to use, and above all they limit freedom of movement. To justify routine bed- side use, the device must be as light as possible but not overly so (see below in this section), arranged on a cart, which is mandatory for easy access to the patient. The idea of having to transport the device should in no case be a physical or psychological obstacle.
The unit should be economically accessible, since the hospital already has sophisticated, high- quality units in the radiology department.
A basic point is that the unit must be compact and smooth, without prominent buttons, or it will not be possible to efficiently disinfect it. The key- board should have flat pressure keys. A volumi- nous unit with prominent buttons will certainly be a godsend for germs. The apparatus we have used for several years is highly compact, weights 13 kg, has a 36-dm
3volume, with advanced esthetics, a flat and easily washable front, with excellent image definition. This technology, which was available before 1994, has contributed to saving countless situations.
Ultraportable »pocket« ultrasound units exist now. One was created many years ago, in a form that we found rather primitive, weighing 3.5 kg and battery-powered [1, 2]. This very rudimentary unit was nevertheless used in the helicopter and was originally, to our knowledge, something of a world premiere [3]. The modern miniature units CHAPTER 2
