La curva OS con un plateau apparente che inizia a circa 3 anni è coerente con quella osservata nei singoli studi di fase II e III di ipilimumab nel melanoma avanzato. La OS mediana e il tasso di sopravvivenza a 3 anni
per i pazienti già trattati erano rispettivamente 10,7 mesi e 20%.

Caso Clinico – trattamenti integrati
• Agata, 55 anni alla diagnosi, fenotipo II, assenza di ustioni solari in anamnesi.
• 2000: exeresi formazione pigmentata regione subungueale (incisionale) secondo dito mano sinistra, con esito istologico di “nevo melanocitico”.
• Successive ulteriori biopsie con esito analogo fino all’onicectomia radicale nel 2003 con ulteriore diagnosi istologica di benignità.
• Revisione preparati istologici (IDI Roma e INT Milano) con conferma della
diagnosi (no melanoma!)
Settembre 2016: a seguito del riscontro di positività del s.o.f.
(Paziente asintomatica, inserimento in programma di screening) pratica pancolonscopia: “alla flessura splenica formazione polipoide di 14 mm di diametro, di aspetto irregolare e superficie mammellonata, priva di pigmento”.
•Asportazione con tecnica della mucosectomia.
•Esame istologico: compatibile con localizzazione da melanoma. Profilo molecolare: BRAF WT.
Novembre 2016
TAC encefalo - torace - addome completo con mdc:
“Voluminosa formazione ovalare, iperdensa, delle dimensioni di
circa 35 mm x 25 mm in sede ascellare sn, riferibile ad
adenopatia”.
PET:
“accumulo linfonodale in sede
ascellare sn (SUV 4,2) ”.
• Dicembre 2016
Linfoadenectomia ascellare sn ed emicolectomia sn laparoscopica.
Es. istologico: “localizzazione di metastasi da melanoma in 3/18 linfonodi ascellari, coalescenti; sezione di parete intestinale senza residui di malattia”.
• Marzo 2017 PET: «patologico accumulo interessante vasta tumefazione in sede mesogastrica paramediana dx» ; TC addome mdc: “ ispessimento delle pareti dell’ultima ansa ileale”;
• Marzo 2017 (altra Sede Ospedaliera): laparoscopia esplorativa con evidenza di carcinosi peritoneale da melanoma; la Paziente viene rimandata al suo domicilio (1200 km in ambulanza).
• Nonostante lo stadio di malattia, la Paziente mantiene un discreto stato generale, fino al maggio 2017…

Laparotomia con resezione intestinale ed exeresi di tre formazioni endoluminali del piccolo intestino; esame istologico:
«lesioni metastatiche da melanoma, BRAF WT»
Maggio 2017 - Luglio 2018:
immunoterapia con pembrolizumab per un totale di 17 somministrazioni.
Nel Luglio 2018 una TAC total body con riscontro tessuto patologico solido amorfo, plurinodulato, a livello della
lingula, con diametro di circa 4 cm e
nodulo satellite di circa 1 cm .
Agosto 2018: toracotomia con exeresi delle formazioni polmonari; esame istologico: «…metastasi da melanoma…»
La paziente riprende il trattamento con pembrolizumab.
Ultimo ciclo il 25.02.2019 e
ultima TC il 06.02.2019, negativa.
