INTRODUZIONE

recidive

Brunschwig A. Complete excision of pelvic viscera for advanced carcinoma; a one-stage abdominoperineal operation with end colostomy and bilateral ureteral implantation into the colon above the colostomy. Cancer. 1948

CONTROINDICAZIONI ASSOLUTE I

• Carcinosi peritoneale

• Metastasi a distanza (epatiche, polmonari)

• Coinvolgimento vasi iliaci alcuni considerano resezioni vascolari con

ricostruzione (interposition graft, bypass femorofemorale) *

Pelvic Exenteration for Advanced Pelvic Malignancies T.M. Pawlik, J.M. Skibber, M.A. Rodriguez-Bigas Annals of Surgical Oncology 2006

* Abdelsattar ZM et al Surgery for locally advanced recurrent colorectal cancer involving the aortoiliac axis:

can we achieve R0 resection and long-term survival? Dis Colon Rectum 2013

CONTROINDICAZIONI ASSOLUTE II

• Estensione tramite forame ischiatico

* Milne, T., et al., Assessing the Impact of a Sacral Resection on Morbidity and Survival After Extended Radical Surgery for Locally Recurrent Rectal Cancer. Ann Surg, 2013

(5)

CONTROINDICAZIONI RELATIVE I

• Metastasi a linfonodi retroperitoneali

• Coinvolgimento parete pelvica alcuni

propongono laterally extended endopelvic

resection (LEER)* =resezione comprendente anche vasi iliaci e muscoli striati

• Invasione diretta della neoplasia di anse intestinali

* Höckel M. Laterally extended endopelvic resection. Novel surgical treatment of locally recurrent cervical carcinoma involving the pelvic side wall. Gynecol Oncol. 2003

(6)

CONTROINDICAZIONI RELATIVE II

• Scarsa compliance paziente gestione stomie

(7)

CONCLUSIONI I

Importanza valutazione e stadiazione preoperatoria:

• TC, RM, PET

(8)

CONCLUSIONI II

Fondamentale planning intervento in base all’imaging

(exenteratio anteriore/posteriore/totale)

(9)

CONCLUSIONI III

(dolore, emorragia, ostruzione intestinale)
