CONCLUSIONI Le evidenze ottenute nel presente studio hanno mostrato come le procedure di ridge

preservation non siano state in grado di prevenire del tutto le alterazioni di profilo e di volume successive all’estrazione dentaria. Questa conclusione integra quanto già visto in letteratura. E’ stato possibile individuare una maggiore predicibilità nel comportamento dell’osso corticale suino rispetto all’innesto cortico-spongioso.

L’innesto cortico-midollare pre-idratato e collagenato disattende le aspettative iniziali, ovvero quelle supportate dal razionale biologico: un innesto de-proteinizzato a cui è stato aggiunto del collagene dovrebbe riassorbirsi più velocemente di un innesto di medesima natura ma che non espone gli opportuni recettori per gli osteoclasti. Di fatto, i dati preliminari del presente studio suggeriscono il contrario. Il materiale controllo ( osso suino corticale ) si rimodella di più e in modo più predicibile.

Gli indici riassuntivi di altezza e di forma della cresta alveolare, se messi in relazione con le variabili anatomiche e le variazioni percentuali delle stesse nel tempo, consentono una visualizzazione immediata di quello che è il comportamento generale dell’alveolo in seguito a procedura di ridge preservation. Il dato tridimensionale, rispetto alle misurazioni lineari, fornisce un’indicazione clinica immediata.

L’utilizzo della misurazione indiretta del volume alveolare, tramite acquisizione con tecnologia CBCT dei modelli in gesso, è una tecnica poco invasiva e di facile esecuzione ma limitata nella sua performance dalla mancanza di distinzione tra tessuti duri e molli.

Servono senz’altro studi con una maggiore popolazione e periodi di follow-up dilatati per l’eventuale conferma di questi riscontri preliminari.
