L’association François Aupetit, le Groupe d’Etudes Thérapeutiques dans les Affections Inflammatoires Digestives - GETAID et le groupe Remind ont la tristesse d’annoncer le décès d’Arlette Darfeuille-Michaud, Professeure de Biologie Cellulaire à l’Université d’Auvergne.
Arlette Darfeuille-Michaud a été pionnière dans la recherche consacrée à la maladie de Crohn. Elle a été à l’origine de la découverte d’une bactérie aux propriétés singulières qui pourrait jouer un rôle clé dans cette maladie.
Depuis sa région d'Auvergne - Clermont-Ferrand, la qualité de ses travaux, la clarté de ses exposés lui ont valu la reconnaissance de ses pairs, dans le monde entier.
C’était une chercheuse infatigable et enthousiaste. Elle a bâti une équipe de recherche dynamique qui poursuit les travaux qu’elle a initié. Ses travaux ont été particulièrement soutenus par l'afa et ses membres se souviendront de ses interventions modestes à l'occasion de la remise de bourses lors de l'assemblée générale annuelle.
Elle avait regagné également le Comité scientifique de l'afa depuis quelques années
apportant son savoir immense. C’était une amie enjouée, attentive aux autres, drôle, profondément humaine.
Nos pensées vont à sa famille et ses enfants qu'elle quitte beaucoup trop tôt.

Probiotici e Disturbi funzionali
I Dolori Addominali Ricorrenti (Pediatric Functional Abdominal Pain): dolori addominali parossistici, con ricorrenza almeno mensile, per almeno tre mesi consecutivi, della durata usualmente inferiore alle tre ore
e tali da modificare le normali attività del bambino (Apley J and Naish Arch Dis Child 1958).
Sono presenti tra l’8 ed il 25% dei bambini in età scolare.

Probiotici e NAFLD
La dieta ricca di grassi e zuccheri induce infiammazione con attivazione dell’immunità innata e quindi anche di NAFLD.
Il ripristino della normale flora intestinale riduce la permeabilità intestinale, incrementa la produzione di acidi grassi a corta catena (SCFAs)
e di ormoni intestinali anoressizzanti (GLP-1, GLP-2), incrementa la sensiblità all’insulina.
Un ciclo breve di VSL#3 migliora significativamente la steatosi epatica e il BMI nei bambini affetti da NAFLD. L’effetto a lungo
termine di questa terapia deve ancora essere valutato.

Non ci sono evidenze che dimostrano che i Probiotici sono superiore al Placebo o agli Aminosalicilati nel mantenimento della remissione nella
Malattia di Crohn ottenuta con terapia medica o chirurgica.
- Tutti gli studi dedicati all’uso di Probiotici nella Malattia di Crohn mostrano delle significative limitazioni.
- Nella Rettocolite Ulcerosa molte specie sembrano mostrare un buon trend nell’induzione della remissione e nel suo mantenimento.
- Nella pouchite le evidenze sembrano supportare l’impiego di VSL#3 nel
mantenimento della remissione.
Nei differenti studi sono stati utilizzati differenti ceppi microbici.
Ogni ceppo potrebbe condurre differenti effetti giustificando l’inconsistenza dei
risultati ottenuti.
Allergia alimentare - La posizione EAACI
PAZIENTI AD ALTO RISCHIO
(Pazienti con almeno un parente con storia di malattia allergica)
Ci sono scarse evidenze che supportano l’efficacia dei probiotici nel trattamento
delle allergie alimentari.
Quattro RCT non hanno mostrato benefici nei confronti della sensibilizzazione verso
allergeni alimentari.

Una systematic review ha trovato insufficienti evidenze di beneficio nelle formule contenenti
probiotici.
Due systematic reviews ed un RCT non hanno trovato benefici nell’uso di probiotici in queste
popolazioni.
