ABSTRACT
Peripheral intravenous (IV) catheter insertion, the most common invasive hospital procedure performed worldwide, is associated with a variety of
complications and an unacceptably high overall failure rate of 35% to 50% in even the best of
hands. Catheter failure is costly to patients, caregivers, and the health care system. Although
advances have been made, analysis of the mechanisms underlying the persistent high rate of
peripheral IV failure reveals opportunities for improvement.

PIV Problems…
•
High rates of failure due to low use of echo guide
Change your point of view…
•
Periferical veins are same important like other organ
•
They must preserving to avoid big problems in
future, discomfort and pain
Preserve for the future
Answer….
•
The right CVC at the beginning.
•
The right CVC for the right patients
•
The longest life for device….
The right VAD…
•
Open end catheter for intra hospital use, blood sampling, CVP measure
•
Valved catheter for long term therapy, home use, chemotherapy
2 2
And then… Quality of life
How VAD change your life?
What type of service: infusion team
•
Organize a proactive
approach to hospitalized patient.
•
Dedicated ambulatory and personnell for different type of patients (Surgical,
Medical, ICU etc.)
•
Take care of CVC’s after
implantation

Take at home messages:
•
Preserve patient’s veins reduce cost, improve QOL and preserve future treatment
•
Pac with ecoguided technique is an easy and safe device for long term endovenous therapy
•
PIV are problematic with high rates of complications and placement failures
•
PICC is a usefull device because is easy, safe as Port but less invasive so usefull in short and medium therapy
•
