Cos'è la traslocazione del gene EML4-ALK
✔
Il gene EML4-ALK è il risultato di un riarrangiamento cromosomico in cui, a causa di eventi casuali, una parte del gene EML4 (la sua porzione N-terminale) si stacca e si unisce alla porzione del gene ALK, contenente il dominio chinasico.

Il riarrangiamento del gene ALK induce la produzione di una proteina anomala che stimola la proliferazione cellulare.
✔
Riguarda solo il 3-5% dei pazienti con diagnosi di adenocarcinoma polmonare.
✔
Si riscontra generalmente nei non fumatori di sesso femminile.

DISCUSSION
“The dual inhibition of ALK and ROS1 by the same small molecule is probably due to structural similarities between these two closely related tyrosine kinases.”
“… the ALK-rearranged and ROS1-rearranged disease subtypes were both highly responsive to crizotinib, with similar times to the first response (median, 7.9 weeks for both) and similar response rates (61% and 72%, respectively).”
“In the ALK expansion cohort of 143 patients, the median duration of response was 49.1 weeks, and the median progression-free survival was 9.7 months.22 In contrast, the estimated median duration of response in the ROS1 cohort was longer, at 17.6 months (75.9 weeks), and the
median progression-free survival was 19.2 months.”
In conclusion, ROS1 rearrangement defines a second molecular subgroup of NSCLC for which crizotinib is highly active. In the majority of patients, crizotinib induced durable clinical responses and was associated with grade 2 or lower toxic effects.
These results highlight the importance of screening for this genetic alteration in patients with advanced NSCLC.

Indicazioni Terapeutiche
Alecensa in monoterapia è indicato per il trattamento in
prima linea di pazienti adulti affetti da carcinoma polmonare
non a piccole cellule (NSCLC) in stadio avanzato positivo per
la chinasi del linfoma anaplastico (ALK). Alecensa in
monoterapia è indicato per il trattamento di pazienti adulti
affetti da NSCLC ALK-positivo in stadio avanzato
precedentemente trattati con crizotinib.
Conclusioni
●Alla luce dei risultati dello studio ALEX appare evidente che nei prossimi anni il trattamento di prima linea dei pazienti con riarrangiamento ALK non potrà prescindere dall'impiego degli inibitori di terza generazione.
●Il Crizotinb sarà l'unico farmaco disponibile nei pazienti con mutazione di ROS 1, per cui sarà importante valutare la presenza del gene sui campioni istologici al momento della diagnosi.
●L'ampliamento della conoscenza dei meccanismi di resistenza grazie al supporto delle nuove metodiche molecolari consentirà lo sviluppo di nuovi farmaci più potenti ed efficaci.
●Di fondamentale importanza sarà la realizzazione di nuove strategie terapeutiche e la valutazione di possibili regimi di combinazione (terapia molecolare e chemioterapia) al fine di ottimizzare il controllo della malattia con il raggiungimento della “cronicizzazione”.
